How does the Acapella Breathing Device work?

The Acapella Breathing Device by Smiths Medical directs exhaled air through an opening, which is periodically closed by a pivoting cone. As air passes through the opening, the cone alternately closes and opens the airflow path. The result is a vibrating pressure waveform. For most users, frequencies between 0-30 Hz are obtained while maintaining positive pressure throughout the exhalation process. This is referred to as vibratory PEP therapy.
Through adjusting the dial, the frequency of the vibrating pressure waves and the resistance of the opening changes.

Acapella Respiratory Device indications:

  • Acapella is intended for use as a single patient-use device that provides Positive Expiratory Pressure (PEP) Therapy for patients who have Cystic Fibrosis, COPD, asthma, and lung diseases with secretory problems and patients with atelectasis.
  • It may also be used simultaneously with nebulized aerosol drug delivery.
  • Green: Acapella DH is recommended for patients able to maintain an expiratory flow of 15 liters per minute or greater for 3 seconds.
  • Blue: Acapella DM is recommended for patients capable of less than 15 liters per minute for 3 seconds.

Contraindications of Acapella Medical Device

Although no absolute contraindications to the use of PEP Therapy have been reported, the following should be carefully evaluated before a decision is made to initiate therapy:

  • Using this device at very high pressure can cause problems. Patients who are sensitive to high lung pressure might experience these issues:
  • Breathing becomes harder and may lead to not getting enough oxygen.
  • The pressure inside the head increases.
  • Problems with the heart and blood circulation.
  • Reduced blood flow back to the heart.
  • Swallowing air, which can lead to vomiting and inhaling stomach contents.
  • Feeling trapped or uncomfortable.

What to buy with Acapella Device for Breathing

How to use Acapella Device?

Initial Settings -

  • For the first time using the Acapella DM & DH Vibratory PEP Therapy System, start by turning the frequency adjustment dial counterclockwise to the lowest resistance setting.
  • Tell the patient to relax and take a deep breath, a bit more than their regular breath but not the fullest they can.
  • Ask the patient to exhale comfortably but not too forcefully through the device until they reach a point called functional residual capacity (FRC). The device will vibrate during exhalation.
  • The patient should aim to exhale for 3-4 seconds while the device vibrates if they can't, turn the dial clockwise to make it a bit harder to exhale.
  • Adjust the dial until you achieve the right balance, which is typically a ratio of inhaling to exhaling of 1:3 to 1:4.
  • If you can't find the right balance, you might need a different device meant for different airflow.
  • Once you find the proper setting, you can fine-tune it by asking the patient to exhale harder or softer or making small adjustments to the dial.
  • It may take a few tries to get it just right for the patient's needs.

Procedure for the User -

  • Make sure the dial is set as your clinician advised.
  • Sit with your elbows on the table for comfort.
  • Gently place the mouthpiece in your mouth.
  • Keep a tight seal on the mouthpiece during exhaling.
  • You might need a nose clip if your clinician suggests it.
  • If you're using a mask, wear it snugly over your nose and mouth.
  • Breathe the way your clinician told you, taking a bigger breath than usual but not filling your lungs completely.
  • Hold your breath for 2-3 seconds.
  • Exhale actively but not too forcefully through the device, making it last about 3-4 times longer than your inhalation.
  • Do 10-20 PEP breaths as your clinician recommended. After that, remove the mouthpiece and do 2-3 huff coughs if you need to clear mucus. Your clinician can guide you on coughing properly.
  • Repeat steps 2 to 7 as directed by your clinician.

How to Clean Acapella PEP System

As needed, detach the mouthpiece, then soak the parts in warm, soapy water, rinse, and dry. Drain the device by placing the unit with the mouthpiece end downward or by resting the unit on its side.

Estimated Device Lifetime

Assuming the manufacturer-recommended cleaning protocol is followed, the Acapella Medical device should have a useful life of six (6) months under normal and customary usage. The six (6) month usage duration is measured from the date of initial use.
